Procedure for Requesting an Accommodation
-
If a qualified applicant for admission to the school or a student of the school anticipates a need for accommodation to complete a course of study or to participate in a school program or activity, the request for accommodation should be brought to the attention of the Regional Medical Programs Coordinator.
Reasonable accommodations will be considered on a case-by-case basis for individuals who meet eligibility under applicable statutes. Any person expecting to need accommodations should request them in writing to the Regional Medical Programs Coordinator two weeks prior to beginning the program, as some accommodations may not be considered reasonable and may impact an applicant’s ability to complete all components of the program.
A reasonable accommodation must accompany an evaluation, documentation of a disability, and/or recommendation(s) from a licensed mental health professional and must have occurred within 1 calendar year of the request. Recommendations from other post-secondary institutions will not be accepted. A candidate will be notified of the accommodation request decision within two weeks of the completed request.
The Regional Medical Programs Coordinator, in conjunction with the appropriate faculty and, if necessary, a medical evaluator retained by the school will determine the feasibility of the requested accommodation, considering various factors including, but not limited to, the nature and the cost of the accommodation, the availability of funding, and whether the accommodation will impact the fundamental nature of the course or program. The applicant/student will be informed of the school’s decision on the accommodation request.

ACCOMMODATIONS REQUEST FORM
School of Practical Nursing – Accommodations Request FormName: ______________________________________ Date Submitted: ______________________
Please submit attached and current (within 2 years of the date submitted) documentation provided by a qualified professional with the appropriate credentials, training, and experience to identify the specific disability diagnosed and to make recommendations for appropriate accommodations. Evaluators should have no personal relationship with the individual being evaluated.
The evaluator’s report must provide clear evidence of a disability, including a statement describing the condition, how the condition was diagnosed, a description of current functional limitations and if pertinent, the prognosis of the condition. The documentation should contain the evaluator’s recommendations concerning specific disability-related accommodations, starting clearly how the recommended accommodations are logically related to functional limitations.
The school may also require periodic updates for the continued provision of accommodations if a disabling condition is not stable (e.g., psychiatric disorders).
An Individualized Educational Plan (IEP), 504 Plan, or high school exit summary of performance may be submitted; however, such documents must contain or be accompanied by the testing and/or other diagnostic evaluation upon which these plans are based; the plans, alone, do not constitute sufficient documentation to support a request for accommodations from the school.
Although the school desires to ensure a smooth admission and transition to our learning environments, the school is not obligated to adopt the specific recommendation of any outside entity.
A recommended accommodation will not be authorized if the accommodation would create an undue hardship for the school or compromise the essential nature of a program or activity or provide an unsupported advantage to one applicant over another in the admission process. All requests are reviewed on a case-by-case basis and any questions or concerns should be discussed with the Program Coordinator once a request determination has been rendered.
Documentation should be mailed or faxed to the confidential attention of
Heidi Mix, Program Coordinator

Documentation will be kept confidential and shared only on a need-to-know basis in accordance with the requirements of all applicable laws.
